New Service Request

Welcome to the Sundale Mutual Water Company Community.  Please contact our office by phone -or- you can print out the Service Application and Agreement (if available) to apply for service. We look forward to serving your water needs.

 

* New Water Service Customer - Please fill out the "New Customer Application & Meter Contract" to set up the meter account.

 

* New Tenant/Renter Water Service Customer - Please fill out the "Tenant/Owner Agreement" to set up the renter on the account.  If as a landlord you would like the property managment to be the point of contact, please fill out "Tenant/Owner Agreement for Property Managers"

 

* Monthly Credit Card Application is optional.  Please fill out if you would like to have your water bill paid automaticly (with a credit card) each month.
